KCEOC is seeking a qualified individual to fill the Homeless Case Manager position at the Main Office in Gray, KY. Minimum qualifications are a human services, sociology, social work, psychology, or similar field degree required; One year work experience in human services required; case management experience extremely helpful; working knowledge of computer programs; supervisory experience helpful; willing to work flexible schedule, including evenings and weekends. The specific responsibilities of this position are: Develop, implement, monitor and/or revise goal plans to move the homeless into an independent housing option; make referrals to and advocate for resources and access to services; Maintain all client records and complete required paperwork within the required timeframes and according to agency and HIPAA guidelines; Provide ongoing encouragement to clients during regular, individual meetings; Provide and procure support to meet the individual’s assessed needs and abilities, and other duties as assigned.
